Apply for a Photo ID Card

Photo ID application form (PDF)

With heightened security measures in place at courts, correctional institutions and other institutions across the province, lawyers and paralegals of the Law Society of Ontario are encouraged to purchase Law Society Photo ID cards.

How to apply:
Lawyers and paralegals can apply quickly and easily for a voluntary Photo ID card by printing the Photo Identification Card Application Form and mailing it in with a passport sized photo to the Law Society. The photo identification cards are available for a fee. A 3 year photo identification card is available for $15.00 (HST included), a 5 year photo identification card is available for $25.00 (HST included) and a 10 year photo Identification card is available for $50.00 (HST included). The purchase of a 10 year photo Identification card entitles a licensee to a free replacement card if their card is lost or stolen during the 10 year period.

The photo submitted must be no older than three months (black and white or colour) and must be certified by another Law Society lawyer or paralegal whose rights and privileges are not suspended.

The credit-card size photo identification card identifies the carrier as a licensed lawyer or paralegal of the Law Society of Ontario. It is available only to lawyers and paralegals whose rights and privileges are not suspended. The photo identification card is the property of the Law Society. The card must be returned to the Law Society upon demand, surrender or revocation of licence.

Lawyers and paralegals are also welcome to apply in person between 9:00 a.m. and 5:00 p.m., at the Law Society's Client Service Centre window. Please bring at least one piece of photo identification when applying in person. Acceptable photo identification includes a passport, driver’s licence, candidate card or an expired Law Society photo identification card.
